     I, Jayita Karmakar, a student of your school of class- IX (Roll-46, Sec-A), with due respect, beg to state you that I filled up Sabuj Sathi form on 10/07/2018 and a cycle was allotted for me. The cycles were distributed on scheduled date from Fulia Kishan Mandi. But on that I had terrible headache and I was bed-ridden for the full day. So, I could not go to receive the cycle. Therefore, I request you to give me another chance so that I get the cycle. If I get the cycle, it will be very much helpful for me to carry on my study. My doctor’s prescription’s photocopy is attached to this application to testify the fact.
