Sub-- Application for issuing a duplicate Ration Card

Sir,

     I, Nanda Gopal Nandi, S/O- Tilok Gopal Nandi, beg to inform you that I am a resident of Bablaban of Haripur Gram Panchayet. But unfortunately, I have lost my Ration Card bearing my name while drawing weekly ration from the ration shop of our locality. My Ration Card No. is—WB/042/B. All my efforts to trace the card having failed, I find no alternative to appeal to you.

    I shall be very much thankful if you issue me a duplicate Ration Card at your earliest realizing the importance of it. I am ready to pay necessary fees for it. A photocopy of my lost ration card is also attached to this application.
