     I, Ruma Mondal, a Saving Bank Account holder of your branch, beg to state you that my Account Number is- 0836010101257 and few days ago I lost my A/C Pass Book of the same account while making its photocopy from a local store. But now I need it to withdraw money from my account very urgently to meet my study expenses.

     Under such circumstance, I earnestly request you to issue me a duplicate A/C Pass Book at your earliest considering my urgency so that I can run my study smoothly. The photocopies of my lost pass Book and my voter id are attached to this application so that it becomes easy for you to trace my details out.
